Abstract : Background: Resin infiltration (RI) material ICON is used in treating early proximal caries lesions, as it depends on a micro-invasive infiltration technology. Aim of the Study: To evaluate the effect of the ICON RI on induced early proximal caries lesions in vitro by comparing the surface roughness and hardness before and after treatment with the RI using Atomic Force Microscopy (AFM) and Automated Microhardness Testing System (AMHTS) (Vickers Hardness). Also, to evaluate the artificial early proximal caries before and after application of ICON RI using Scanning Electronic Microscope (SEM) in vitro. Materials and Methods: Twenty seven human sound premolars extracted for orthodontic reasons were sectioned in a buccolingual direction into two halves using isomet low speed saw, creating 54 specimens in total. Each specimen was immersed for 2 weeks in demineralizing solution to induce caries-like lesion. Only teeth with International Caries Detection and Assessment System (ICDAS) codes 1 and 2 were selected. The specimens were divided randomly according to test methods into either AFM (n=25), AMHTS (n=25) or SEM (n=4). The AFM group examined the demineralized enamel before and after treatment with ICON. The AMHTS also examined the demineralized enamel before and after treatment with ICON by transversal Vickers microhardness. Also, 4 samples were sent for examination with SEM. The collected data were submitted to statistical analysis using Wilcoxon-Signed Rank test with level of significance among the groups at p<0.05. Results: The mean average surface roughness (Ra) and Root mean square roughness (Rq) values of the demineralized enamel treated with ICON were significantly higher than the untreated lesions (p<0.001). Also, the mean Vickers hardness values for the demineralized enamel treated with ICON was significantly higher than the untreated lesions (p<0.001). In the SEM, the demineralized enamel surface showed irregular pitted rough enamel surface with multiple craters of variables depths with destruction of enamel rods and dissolution of the enamel crystals. After ICON application, the surface showed complete blockage of the enamel rods with RI, irregular rough uneven topography and the presence of a hypermineralized layer that completely covered the enamel surface. Conclusion: Resin infiltration application on proximal incipient caries lesions increased the surface roughness and hardness significantly.
