Accuracy of Bracket Positioning Using Magnifying Loupes

Abstract : Dentistry is a visually demanding profession. Using dental magnification devices is assumed to promote higher quality treatment. Magnification devices have been used in different dental professions. In Orthodontics, an accurate placement of the orthodontic brackets is one important step to get a well-finished orthodontic treatment. However, there is very scarce literature about the use and benefits of magnification aids in orthodontics. The objective of this study was to identify if there are any significant differences in the accuracy of brackets positioning when using magnifying loupes and not using them. Stone models with mild crowding, vertical and rotation discrepancies were developed and used to yield duplicates and used for the bonding procedure. Nineteen orthodontists and 19 residents were asked to bond brackets on casts mounted on a simulation unit on the dental chair to simulate the clinical situation as much as possible. They were asked to bond the brackets twice. The first time without the use of magnification loupes and the second time with the use of the magnification loupes. The participants were instructed to bond the brackets at the center of the clinical crown in the mesio-distal dimension, along the long axis of the tooth and at a specific distance from the incisal or occlusal edge. Before they used the magnification devices, a brief verbal explanation about the use of loupes were given to them to assure that the participants were comfortable while using them. Then the casts were sprayed using CEREC Optispray. Then they were scanned using 3D Dental Scanner and the files were imported as stl. file format. Blender software 2.79 was used to measure the errors in three dimensions (vertical, angulation and mesio-distal). The association between bracket placement errors among each tooth and among teeth groups with different groups of participants were tested using one way ANOVA and student’s t-test. The mean time difference in bracket among participants was reported and tested among participants based on their experience and magnification loupe using by one way ANOVA test. There was no significant difference in the accuracy between using the loupes and not using them (52.9%±8.3, 50.6%±6.3, P>0.05). The residents took significantly longer time than the orthodontists to complete the procedure when not using the loupes (46.7±26.9min, 30±7.9min, P<0.05) while there was no significant difference between the two groups when using the loupes. The accuracy of orthodontic brackets positioning does not appear to be affected by using magnifying loupes. However, using magnification loupes in the Orthodontic field is quit new concept and not commonly used on the daily Orthodontic practice. The results achived could be justified by the relatively long learning curve of the magnification loupes. In this regard, only 13% of the participants (5 out of 38 participants) in this study used magnification loupes at least once in their daily orthodontic practice. However, not all the 5 participants were completely familiar and used to using the magnification devices during their daily orthodontic practice. Another factor might lead to the insignificant difference between the orthodontists and the residents is that not all the orthodontists were engaged in the study conversely to the residents who were excited and more engaged in the study and more welling to learn new things.
